FAQ

1. What should I bring with me to my evaluation appointment?

Bring your insurance card(s), a photo ID, a list of your medications, and any assistive devices you use, such as glasses or a hearing aid.  If you usually take medication during the time you will be here, bring that as well.

2. Do I need a doctor’s order?

Only if we are billing Medicare; otherwise, no physician’s order is needed.

3. What should I expect during my evaluation appointment?

You will meet with the doctor for an interview, and do any testing he/she prescribes.  Some of the tests are pencil/paper that you do on your own, but others are administered by a psychometrician (a person who administers tests).

4. Do you take insurance?
